Additional benefits with this membership include 15% savings on all laser services, 5% savings on all injectables, plastic surgery, weight loss management, and hormone replacement therapy.

By signing up for the Premier Platinum membership, you agree to be charged $300.00 on the 1st of every month until the end of this membership agreement has concluded. The monthly fee of $300.00 is for the Premier Platinum membership fee and is not available as credit. This membership has a commitment of 6 months. At the end of the 6 months, you will automatically begin a new membership cycle unless you cancel with at least 24 hours before the renewal date. (6 months from sign-date). Unused monthly treatments expire the last day of each month. "Treatment Offerings" are not available as cash value and may not be used as 'credit' towards other procedures and treatments. Offers and benefits included within the Premier Platinum membership cannot be transferred or used by another patient. By signing up for this membership, you understand and agree that Fulcrum Aesthetics has a zero refund policy. Early termination of a membership voids all promotional pricing, and any services already received will be charged at the standard retail rate, with the remaining balance due upon cancellation.
